How to Apply For EdChoice Expansion Scholarship

Step 1 - Account Setup and Income Verification

Visit safe.ode.state.oh.us/portal to create an account and submit income verification.

Appropriate documents are outlined in the Online Income Verification Process Form depending on your employment. If the reviewers need more information, the EdChoice Office will reach out to you via email once they review it. 

Parents are encouraged to utilize the video tutorial on how to use the Income Verification website or download the Guide for Income Verification.

Step 2: Upload Documentation and send to st. Angela’s

Fill out the EdChoice Request Form (one for each child) and send request form and all requested documentation listed below to Lisa Whelan and Jennifer Fitzpatrick.

1.    EdChoice Request Form 2023-24

2.   Copy of Birth Certificate of student

3.   Proof of Address/Utility bill (gas, water, electric). If utilities are not in parent’s name, see “Address Verification” section on Request Form.
